How to read this
The red mark is the published kill line — the price that would prove the thesis wrong. The dot is where the name closed on 9 October 2026; a dot left of the mark has closed through its line.
Distance is drawn on a square-root scale, so close calls get the room. Past 8% a row reads well clear, with a hollow dot to show the figure is off the drawn scale. Rows run tightest first.
Kill line
A weekly close below $17, the lower edge of the range the shares held through September; separately, third-quarter revenue falling more than the guided 1–2% sequentially or a cut to the $231–233 million 2026 revenue outlook.
Open pick Medium conviction, opened at $18.41. Since then +5.2% against the S&P 500's +1.9% (+3.3 pp). Kill line at open: $17.00 (weekly close); graded by at the latest.

SI-BONE's 15% revenue growth, its first spinal implant and higher Medicare payments for complex fusion support a recovery case for the shares. The third-quarter report, which aggregators place around 9 November 2026, settles it by showing whether revenue holds the raised $231–233 million outlook for 2026. The business anchor remains the company's 3 August 2026 release: second-quarter revenue of $56.0 million, up 15.2% year over year, and adjusted earnings before interest, taxes, depreciation and amortization (EBITDA) of $2.8 million.
Nothing in the business has changed since the 7 October 2026 update. As of 9 October 2026 the company has not announced its third-quarter report date and has not disclosed a first Onyx surgery. The price has moved: the shares closed at $18.86 on 8 October 2026, against $19.81 on 5 October 2026, the day Jefferies raised its price target to $25 from $23.
The narrative is maturing. Onyx, the 1 October 2026 Medicare payment change and 15% growth are all public, and the shares have given back part of the 5 October 2026 gain without new company news. The next new business fact is either a first Onyx surgery or the third-quarter report itself.
Bullish and bearish views on SI-BONE, Inc.
The bull case on SI-BONE, Inc. (SIBN), in brief: Growth has a published hurdle. The bear case: Accounting profit is still absent.
Bull case
- Growth has a published hurdle. SI-BONE's 3 August 2026 release reported second-quarter revenue up 15.2% with a 79.5% gross margin and raised 2026 revenue guidance to $231–233 million. Active US physicians rose 19% to 1,715 in the same quarter (8-K exhibit).
- Onyx takes the platform into the spine. The FDA cleared iFuse Granite Onyx on 28 September 2026. The device targets screw loosening at the ends of spinal fusion constructs, which the company says occurs in up to 27% of cases, and the company described a phased fourth-quarter launch beginning as early as October (company release via BioSpace).
- Reimbursement changes are live. The Centers for Medicare & Medicaid Services' fiscal-2027 hospital payment rule, published 4 August 2026, took effect on 1 October 2026 and creates new payment groups for complex fusion procedures using iFuse Bedrock Granite. Management said at the 14 September 2026 Morgan Stanley conference that the larger commercial effect should come in 2027.
- The balance sheet funds the plan. The 3 August 2026 release reported $145.9 million of cash and marketable securities and guided 2026 adjusted EBITDA to be positive.
- Analyst targets sit above the shares. Jefferies' target is $25 as of 5 October 2026. A nine-analyst survey cited by ad-hoc-news on 28 September 2026 put the average target at $25.00, in a $20–32 range.
Bear case
- Accounting profit is still absent. The 3 August 2026 release reported a second-quarter net loss of $4.1 million under generally accepted accounting principles, against adjusted EBITDA of $2.8 million.
- The third quarter is guided lower. Management guided third-quarter revenue down 1–2% from the second quarter's $56.0 million because of summer vacation timing. A larger drop would weaken the growth case.
- Onyx has no revenue timeline. The 28 September 2026 release gave no revenue estimate, and no first surgery had been disclosed by 9 October 2026. That makes Onyx a 2027 contributor at most.
- Insider sales cluster into strength. SEC Form 4 filings show director Jeffrey Dunn sold 20,000 shares at a weighted average of $18.47 around 1 October 2026 under a 10b5-1 plan, and two officers sold 8,182 shares combined on 2 October 2026 and 5 October 2026, mostly to cover taxes on vesting restricted stock units. No insider purchases appear in the same filings.
- A small recall is open. SI-BONE disclosed on 11 September 2026 a recall of 98 TORQ implant units with an estimated cost of up to $150,000.
Price setup
The shares closed at $18.86 on 8 October 2026, 11.9% below the 52-week high of $21.40, according to adjusted daily closes. They rose 5.7% over the three months to that date, and the 14-day relative strength index stood at 57.1, a reading between oversold and overbought.
The recent path is short. The close was $19.81 on 5 October 2026 after the Jefferies target raise, $19.55 on 6 October 2026 and $18.86 on 8 October 2026. The shares held above roughly $17 through September, which makes $17 the lower edge of the recent trading range. The 52-week high of $21.40 is the upper reference.
Crowding is limited. Retail attention is low for a company of this size, and the observable supply comes from insider sales filed on Form 4, most of them under 10b5-1 plans or for RSU tax cover. The earnings date is roughly a month away and not yet confirmed.
Catalysts, next 30 days
- ~20 October 2026, est.: Company announcement of the third-quarter report date. SI-BONE reported the third quarter of 2025 on 10 November 2025.
- ~31 October 2026, est.: Initial limited Onyx launch, which the company said could begin as early as October. A first-surgery disclosure would give the 28 September 2026 clearance a commercial start.
- ~9 November 2026, est.: Third-quarter 2026 results, just outside the 30-day window. The case turns on whether revenue stays within the guided 1–2% sequential decline and whether the $231–233 million outlook holds.
What would change the view
The case breaks if the third-quarter report shows revenue falling more than 1–2% from the second quarter's $56.0 million, or if management cuts the $231–233 million 2026 revenue outlook. Either result would mean the slowdown is more than summer timing.
A second break would be the Onyx launch slipping into 2027 with no limited-launch surgeries disclosed at the third-quarter report. A widening of the TORQ recall beyond 98 units, or a charge above the up-to-$150,000 estimate, would also weaken the read.
On price, a weekly close below $17 would put the shares under the range they held through September and erase the September recovery.
What it trades with
SI-BONE's revenue depends on US procedure volumes and Medicare payment rules for sacroiliac and spinal fusion, which it shares with other spine-device makers. Its pelvic-trauma revenue runs through the Smith+Nephew distribution agreement signed 23 February 2026, so that partner's sales priorities affect one revenue stream. Recent moves have been company-specific: the largest October move came on the 5 October 2026 Jefferies target raise. No measured correlation with a small-cap or medical-device index is published here.
Notes
- SI-BONE is still loss-making under generally accepted accounting principles; the profitability case rests on adjusted EBITDA, which was $2.8 million in Q2 2026.
- Pelvic-trauma revenue runs through the Smith+Nephew distribution agreement signed 23 February 2026, so the partner's sales priorities govern that stream.
- Quarterly revenue is seasonal: management flags summer vacation timing as a third-quarter headwind, so a modest sequential decline is part of guidance.
- Officer share sales often cover taxes on vesting restricted stock units, and several director sales run under 10b5-1 plans, so recurring Form 4 sale filings are expected.
