How to read this

The red mark is the published kill line — the price that would prove the thesis wrong. The dot is where the name closed on 9 October 2026; a dot left of the mark has closed through its line.

Distance is drawn on a square-root scale, so close calls get the room. Past 8% a row reads well clear, with a hollow dot to show the figure is off the drawn scale. Rows run tightest first.

The thesis

Intuitive Surgical's business is still growing faster than its share price implies. Revenue rose 19% in the second quarter of 2026, yet the shares closed at $415.41 on 8 October 2026, 29.9% below their 52-week high of $592.85. The third-quarter report on 20 October 2026, after the market close, settles the case. It shows whether procedure growth holds inside the 13.5%–15.5% full-year range or slows enough to justify the lower price.

Two things changed since the 7 October 2026 note. The shares rose from $404.76 on 6 October 2026 to $415.41 on 8 October 2026. Barclays started coverage on 7 October 2026 with an Overweight rating and a $485 target, arguing that strong medtech names look attractive after the sector's weakness and that industry fundamentals should stabilise in 2027 (Benzinga; goldesel.de). The company's investor relations calendar confirms the earnings call for 20 October 2026 at 1:30 p.m. Pacific time.

The narrative is maturing: the da Vinci story is widely known, and the Barclays initiation on 7 October 2026 rests on a sector-recovery argument rather than new company results. Barchart measured the shares down 8.4% over the 52 weeks to 30 September 2026, while the S&P 500 gained 15.2%. Expectations going into the report already look reset lower, which is the core of the case.

Bullish and bearish views on Intuitive Surgical, Inc.

The bull case on Intuitive Surgical, Inc. (ISRG), in brief: Revenue and earnings still compounding. The bear case: Guidance implies a slower second half.

Bull case

  • Revenue and earnings still compounding. Second-quarter 2026 revenue was $2.89 billion, up 19% from $2.44 billion. Non-GAAP diluted EPS (earnings per share) was $2.80, against $2.19 a year earlier (company release, 16 July 2026).
  • Procedures, the recurring driver, grew 16%. Worldwide procedures grew about 16% in the second quarter, with da Vinci up about 15% and Ion up about 36% (company release, 16 July 2026). Instruments and accessories, bought for every procedure, brought in $1.73 billion, up 18%.
  • New system cycle is adding placements. The company placed 468 da Vinci systems in the quarter against 395 a year earlier, including 246 da Vinci 5 units against 180 (company release, 16 July 2026). The installed base reached 11,710 systems.
  • Approved uses keep widening. Da Vinci 5 won CE mark approval in Europe for adult cardiac procedures, including minimally invasive coronary bypass, on 29 September 2026 (MDDI). Da Vinci SP won expanded European gynecologic use earlier in September (ad-hoc-news, 16 September 2026).
  • The valuation has already compressed. GuruFocus put the price-to-earnings ratio at 46.6 times on 5 October 2026, against a five-year median of 70.9 times. Analysts expect 2026 EPS of $10.78, up 20.71% (stockanalysis.com, 5 October 2026).
  • Analyst support is broadening. Barclays initiated at Overweight with a $485 target on 7 October 2026 (Benzinga). RBC Capital set a $575 target with a Buy rating on 1 October 2026 (ad-hoc-news), and Barchart counted 22 "Strong Buy" ratings among 31 analysts on 30 September 2026.

Bear case

  • Guidance implies a slower second half. Management guided 2026 procedure growth to 13.5%–15.5%, "closer to the midpoint" (company release, 16 July 2026). That range sits below the 17% of the first quarter and the 16% of the second.
  • The US business is decelerating. An Intuitive executive said on 9 September 2026 that the US business, about 60% of operations, is slowing (Benzinga). The company pointed to a planned 2027 extended-use instrument program as a later support for procedure growth, which places the help outside this report.
  • Quarterly earnings growth is expected to slow. Barchart's consensus of $2.13 per share for the third quarter implies 8.7% growth from $1.96 (30 September 2026), well below the 28% EPS growth of the second quarter.
  • Tariffs weigh on margin. The 2026 gross margin guide of 68.0%–69.0% already includes a tariff cost of about 1.0% of revenue (company release, 16 July 2026). A wider tariff hit would land directly on that range.
  • Antitrust exposure has reopened. Intuitive asked the full Ninth Circuit on 29 September 2026 to rehear a panel decision that revived antitrust claims over remanufactured instruments in Surgical Instrument Service Co. v. Intuitive Surgical (Law360).
  • Earnings reactions have been sharp. The shares fell 14.2% the session after the 16 July 2026 report (TipRanks). A soft US procedure number could repeat that move.

Price setup

The shares closed at $415.41 on 8 October 2026, up from $406.48 on 5 October 2026 and $404.76 on 6 October 2026, in adjusted daily closes. They sit 29.9% below the 52-week high of $592.85 and are up 2.0% over three months. The September rebound began from a close of $351.13 on 9 September 2026.

The 14-day relative strength index (RSI), a momentum gauge, read 64.0 on 8 October 2026. That reflects firm buying into the report without reaching the 70 level usually read as overbought.

Crowding evidence is moderate. The earnings date is eight trading sessions away, and Benzinga's 30 September 2026 pieces framing Intuitive as a "physical AI" leader drew retail attention to the name. The three-month gain of 2.0% and the 29.9% gap to the 52-week high show the shares are not extended against their recent history.

Catalysts, next 30 days

  • 20 October 2026, after the market close: third-quarter 2026 report and call (1:30 p.m. Pacific). The report shows whether worldwide procedure growth stays inside 13.5%–15.5%, how far US growth slows, whether da Vinci 5 placements keep rising from 246 in the second quarter, and whether tariffs force a cut to the 68.0%–69.0% gross margin guide. Some data aggregators list 19 October 2026; the company's investor relations event page lists 20 October 2026.
  • Date not set: Ninth Circuit ruling on Intuitive's petition for full-court rehearing. A denial would send the revived remanufactured-instrument claims back toward trial.

What would change the view

The case breaks if the third-quarter report shows procedures slowing faster than guidance allows. Worldwide procedure growth near or below 13.5%, or a lowered 2026 procedure range on 20 October 2026, would show the share-price discount is justified. A cut to the 68.0%–69.0% gross margin guide, or instruments and accessories growth falling well behind procedure growth, would also weaken it.

On price, a daily close below $351, under the 9 September 2026 close of $351.13 that started the September rebound, would erase the whole recovery and end the thesis. A Ninth Circuit denial of rehearing would add a legal overhang without breaking the operating case on its own.

The case strengthens if procedure growth lands at or above the 14.5% guidance midpoint and US growth stabilises.

What it trades with

Intuitive trades with large-cap medical technology. Barclays' 7 October 2026 initiation explicitly ties its view to a sector recovery, so a further medtech sell-off would pressure the shares even on solid results. The Medtech & diagnostics theme cluster has been heating since late September, but its peers (Illumina, Natera, Grail and other genomics names) are diagnostics businesses, and Intuitive's read depends on its own procedure numbers rather than on that group.

The "physical AI" framing in Benzinga's 30 September 2026 coverage links the shares to robotics funds such as BOTZ. That adds sensitivity to swings in the AI and robotics trade that the hospital business itself does not drive.

Notes

  • Intuitive Surgical pays no dividend; it returns capital through share repurchases, including 0.9 million shares for $0.38 billion in the second quarter of 2026.
  • Intuitive reports procedure growth and system placements each quarter; these operating figures often move the shares more than earnings per share does.
  • Intuitive reports quarterly results after the US market close, so the share reaction shows up in the following session.
  • Intuitive faces continuing antitrust litigation over third-party remanufactured instruments, including Surgical Instrument Service Co. v. Intuitive Surgical in the Ninth Circuit.
