How to read this

The red mark is the published kill line — the price that would prove the thesis wrong. The dot is where the name closed on 19 October 2026; a dot left of the mark has closed through its line.

Distance is drawn on a square-root scale, so close calls get the room. Past 8% a row reads well clear, with a hollow dot to show the figure is off the drawn scale. Rows run tightest first.

The thesis

McKesson's specialty-drug and oncology business is growing fast enough to keep lifting earnings above guidance. The fiscal second-quarter report on 4 November 2026 decides whether the shares keep their record level. The question is whether the company raises its fiscal 2027 adjusted earnings per share (EPS) outlook of $44.20 to $45.00 a second time.

The evidence for the earnings story is strong. The evidence on price is stretched. In the quarter reported 5 August 2026, adjusted EPS rose 20% to $9.93 on revenue up 8% to $105.4 billion. Oncology & Multispecialty revenue rose 33%. The shares closed at a record $1,010.43 on 19 October 2026, up 24.3% in three months, with a 14-day relative strength index (RSI) of 88.3. That reading is far into overbought territory.

The narrative is maturing. The compounding story is widely covered: Benzinga ran 5-year and 20-year return retrospectives on 24 September 2026 and 5 October 2026. Fresh news still arrived in October, including the 1 October 2026 CVS Health contract extension and the 6 October 2026 Option Care Health deal.

Bullish and bearish views on McKesson Corporation

The bull case on McKesson Corporation (MCK), in brief: Oncology and specialty drive the growth. The bear case: The price has run ahead of most targets.

Bull case

  • Oncology and specialty drive the growth. Oncology & Multispecialty revenue rose 33% to $14.2 billion in fiscal Q1 2027 (company release, 5 August 2026), and adjusted segment operating profit rose 41% to $405 million.
  • The core distribution business is widening margins. North American Pharmaceutical revenue grew 5% to $86.8 billion, while adjusted segment operating profit grew 19% to $894 million (5 August 2026 release).
  • Guidance was raised, then reaffirmed. On 5 August 2026 the company lifted fiscal 2027 adjusted EPS guidance to $44.20–$45.00 from $43.80–$44.60, implying 13%–15% growth. It reiterated that range on 1 October 2026.
  • The largest customer is locked in. McKesson and Cardinal Health extended their CVS Health distribution agreements through June 2032, according to Benzinga on 1 October 2026.
  • Home infusion adds a new specialty channel. McKesson will invest about $1.4 billion for roughly 49% of Option Care Health. CD&R takes about 51% in the $5.8 billion, $32.05-per-share buyout announced 6 October 2026. The agreement includes a framework for McKesson to buy CD&R's stake later.
  • Capital returns are large. The company repurchased $2.5 billion of stock in fiscal Q1 2027, including a $2.25 billion accelerated buyback. It also raised the quarterly dividend 15% to $0.94 (5 August 2026 release).
  • Analysts keep raising targets. UBS lifted its target to $1,200 on 16 October 2026, and Leerink Partners raised its target to $1,070 from $1,030 on 7 October 2026 (Benzinga).

Bear case

  • The price has run ahead of most targets. MarketBeat put the average analyst target at $983.29 on 15 October 2026. That is below the 19 October 2026 close of $1,010.43.
  • Momentum is extreme going into earnings. The 14-day RSI stood at 88.3 at the 19 October 2026 close, after a 24.3% three-month gain. The stock is at its 52-week high 11 trading days before the report.
  • Cash flow was negative last quarter. Free cash flow was negative $372 million in fiscal Q1 2027, and GAAP EPS fell $1.10 to $5.15 (5 August 2026 release).
  • Medical-Surgical is shrinking in profit. Adjusted operating profit in Medical-Surgical Solutions fell 20% to $195 million in fiscal Q1 2027. The company warns that the planned separation of that unit, to be named Wellverse, may not happen "on the currently contemplated terms or timeline, or at all."
  • Deal execution adds complexity. The Option Care transaction needs shareholder and regulatory approval and is expected to close in the first half of 2027, per press coverage of the 6 October 2026 announcement.

Price setup

The shares closed at $1,010.43 on 19 October 2026 in adjusted daily closes. That is a 52-week high and 0.0% below it. The three-month gain is 24.3%. The 14-day RSI of 88.3 shows how one-sided the recent buying has been. MarketBeat recorded an intraday 52-week high of $1,003.21 on 15 October 2026, so the 19 October 2026 close set a fresh record.

Crowding is visible in three places: the overbought RSI, a mainstream television endorsement (Jim Cramer called McKesson a "winner" on 15 October 2026), and an earnings date 11 sessions out. Benzinga put the market value at $106.73 billion on 5 October 2026. A daily close below $930 would give back about 8% from the record close and sit below the $983.29 average analyst target.

Catalysts, next 30 days

  • 4 November 2026 (confirmed, after market close; call 4:30 p.m. ET): fiscal Q2 2027 results. Third-party earnings sites compile consensus near $10.73 adjusted EPS on about $110.02 billion of revenue. This report decides the case. The test is a second raise to the $44.20–$45.00 guidance and continued Oncology & Multispecialty growth.
  • First half of 2027 (est.): Option Care Health closing. The deal depends on Option Care shareholder approval and regulatory clearance. Neither date has been set.
  • Date not set: Medical-Surgical (Wellverse) separation. The company has given no completion date.

What would change the view

The case breaks if the 4 November 2026 report shows the earnings engine slowing. That would mean fiscal 2027 adjusted EPS guidance held flat or cut from $44.20–$45.00, or Oncology & Multispecialty revenue growth falling well short of the 33% posted in fiscal Q1. A loss or repricing of a major customer contract would also break it. So would the Option Care deal becoming a drag on cash returns, for example through a reduced buyback.

On price, a daily close below $930 would show the post-September gains unwinding. The shares would then trade below the average analyst target. A sharp drop after the earnings release, even with guidance intact, would show that expectations had moved above what the business delivered.

What it trades with

McKesson trades with the other US drug distributors, Cardinal Health (CAH) and Cencora (COR). Industry news such as the shared CVS Health extension of 1 October 2026 moves all three. As a health-care distributor, it tends to draw buying in defensive rotations, and its link to technology and AI-driven indexes is low. Option Care Health (OPCH) now trades on the spread to the $32.05 deal price. That spread gives a market read on the odds the deal closes.

Notes

  • McKesson's fiscal year ends 31 March; fiscal 2027 runs April 2026 through March 2027.
  • McKesson plans to separate its Medical-Surgical Solutions unit as a standalone company named Wellverse; Apollo Funds bought about 13% of it for $1.25 billion in June 2026.
  • McKesson reports adjusted EPS alongside GAAP EPS and gives guidance only on the adjusted measure.
