How to read this
The red mark is the published kill line — the price that would prove the thesis wrong. The dot is where the name closed on 30 October 2026; a dot left of the mark has closed through its line.
Distance is drawn on a square-root scale, so close calls get the room. Past 8% a row reads well clear, with a hollow dot to show the figure is off the drawn scale. Rows run tightest first.
Kill line
A daily close below $330, which was Barclays' price target before the Q3 report. Also 2027 adjusted EPS guidance in February 2027 that falls below the 2026 range of $18.30–$18.70.
Open pick Medium conviction, opened at $355.59. Since then +2.2% against the S&P 500's −2.5% (+4.7 pp). Kill line at open: $330.00 (daily close); graded by at the latest.

Labcorp's third-quarter beat and raised 2026 guidance support the shares edging ahead of the S&P 500 over the next three months. The Centers for Medicare & Medicaid Services (CMS) tests that view first with final 2027 lab rates, due in November 2026. The company's 2027 outlook, expected in February 2027, settles the case.
Since the 28 October 2026 report, the analyst case has strengthened. HSBC upgraded the shares to Buy from Hold on 5 November 2026, and Seeking Alpha and stockanalysis.com put its target at $410. Mizuho raised its target to $400 from $355 on 4 November 2026. Some wire headlines print the HSBC target as $40, which looks like a typing error.
The narrative is maturing: Labcorp is a well-covered large company, and six brokers raised targets on 29 October 2026 without changing ratings. The HSBC move on 5 November 2026 is the first rating upgrade found in company news since the report.
Bullish and bearish views on Labcorp Holdings Inc.
The bull case on Labcorp Holdings Inc. (LH), in brief: Central lab growth is accelerating. The bear case: Medicare fee cuts arrive in 2027.
Bull case
- Central lab growth is accelerating. Biopharma Laboratory Services revenue was $866.7 million in the third quarter, up 8.5%, or 9.6% organic. Central Labs grew 11.5%, according to the company's 28 October 2026 8-K.
- Orders keep running ahead of revenue. The trailing twelve-month book-to-bill ratio was 1.10 on $3.59 billion of net orders. Backlog was $8.92 billion, up 3.9%, as of 30 September 2026.
- Results beat and guidance rose. Third-quarter revenue of $3.81 billion and adjusted earnings per share (EPS) of $4.78 topped consensus estimates of $3.759 billion and $4.66. On 28 October 2026 the company raised 2026 adjusted EPS guidance to $18.30–$18.70, from $18.10–$18.55.
- Routine testing volumes are growing. Diagnostics Laboratories revenue was $2.95 billion in the third quarter, up 6.5%, or 4.4% organic. Organic requisition volume rose 3.1%.
- Cash flow improved. Third-quarter free cash flow was $412.2 million, against $280.5 million a year earlier, according to the 28 October 2026 8-K.
- Medicare cuts have a yearly ceiling. CMS's fact sheet on the 21 September 2026 preliminary rates says a test's payment cannot fall more than 15% in a year, with reductions phased in from 2027 through 2029.
Bear case
- Medicare fee cuts arrive in 2027. Labcorp put the 2027 impact at about $125 million on its 28 October 2026 call, based on preliminary CMS rates under the Protecting Access to Medicare Act (PAMA). CMS's final rates can differ, and the company plans to quantify the effect in its 2027 guidance.
- Reported profit grew much more slowly. Third-quarter GAAP diluted EPS was $3.22, up 3.3%, while adjusted EPS rose 14.5%. Adjustments account for most of the headline earnings growth.
- Early-stage drug services are flat. Early Development revenue grew 0.4% in the third quarter. Biopharma momentum sits in Central Labs.
- Free cash flow guidance did not move. On 28 October 2026 the company left its 2026 range at $1,240–$1,360 million while raising revenue and EPS guidance.
- Not every broker sees upside. Deutsche Bank's $355 target with a Hold rating, set on 29 October 2026, sits below the 5 November 2026 close of $363.30. As of 6 November 2026, MarketBeat's average target was $369.08 and stockanalysis.com's was $382.25.
- Insiders are selling small amounts into strength. A Form 4 shows Diagnostics president Bryan Vaughn sold 306 shares at $353.70 on 3 November 2026 under a Rule 10b5-1 trading plan, leaving 6,844.9105 shares held directly. Fiscal.ai separately reported a $389,395 insider sale on 3 November 2026 without naming the seller.
Price setup
The shares closed at $363.30 on 5 November 2026, 1.3% below the $368.16 52-week high in adjusted daily bars. They are up 13.3% over three months. The 14-day relative strength index, a momentum gauge often read as overbought above 70, stands at 76.7.
The price has moved within a narrow range since the report. The close on 30 October 2026 was $361.39, and Form 4 filings show transactions at $355.59 on 2 November 2026 and $353.70 on 3 November 2026. MarketBeat reported an intraday high of $373.34 on the day of the HSBC upgrade, so the shares closed well below that peak.
Crowding is moderate. The momentum reading is stretched and insiders have sold in small size. No earnings report is due until about 5 February 2027, so the near-term tests are the CMS final rates and management's conference comments.
Catalysts, next 30 days
- 10 November 2026, 8:45 a.m. ET: Chief Financial Officer Julia Wang speaks at the UBS Global Healthcare Conference, per the company's 4 November 2026 8-K. This is management's first scheduled public appearance since the third-quarter call.
- 24 November 2026: Record date for the $0.72 quarterly dividend, payable 11 December 2026. This is a routine event that does not test the growth case.
- ~30 November 2026, est.: CMS says it will publish final 2027 lab rates in November 2026, without naming a day. The result shows whether the roughly $125 million estimate still stands.
- ~12 January 2027, est.: J.P. Morgan Healthcare Conference, where management may comment on how much of the Medicare impact volume and acquisitions can absorb.
- ~5 February 2027, est.: Fourth-quarter report and 2027 guidance, which settle the case.
What would change the view
The case breaks if 2027 adjusted EPS guidance in February 2027 falls below the 2026 range of $18.30–$18.70. That would show Medicare cuts outweighing volume growth and acquisitions.
Earlier warnings would be final CMS rates or company comments that lift the 2027 impact above about $125 million. A fourth-quarter biopharma book-to-bill below 1.0, or Central Labs growth falling to low single digits, would undercut the drug-development part of the case. A block or long delay on the Millennium Health, Myriad Prenatal Health or BioReference asset deals would remove part of the acquired growth.
On price, a daily close below $330 would break the view. That level was Barclays' target before the third-quarter report and sits below Deutsche Bank's current $355 target.
What it trades with
Quest Diagnostics is the closest read-across because it faces the same CMS rates, so the November final schedule bears on both companies. Fortrea, spun off from Labcorp in 2023, depends on the same clinical-trial spending that drives Biopharma Laboratory Services, though Labcorp's exposure now covers central lab and early development work only. Medicare rate decisions are specific to the lab sector and do not depend on broad market direction.
Notes
- Labcorp spun off its clinical development business as Fortrea in 2023. Biopharma Laboratory Services now covers only central lab and early development work.
- Medicare lab payment rates are reset under PAMA. CMS says reductions run from 2027 through 2029, and a test's payment cannot fall more than 15% in a year.
