Current Thesis
Two weeks past the Q2 print, nothing has arrived that resolves the arithmetic the print created. Revenue of $70.661M on 2026-08-10 landed below the company's own $71–81M guidance floor, and FY2026 guidance was reaffirmed anyway at $350–365M revenue with $80–100M adjusted EBITDA. Against H1 revenue of $114.9M, that leaves $235–250M for the back half — INFERRED from the two disclosed figures, not a company statement — while adjusted EBITDA has to travel from Q1 $(12.7)M and Q2 $(1.2)M to a guided full-year $80–100M. The equity is pricing that swing against a balance sheet carrying $292.4M loans payable net, $83.9M cash and $14.8M of stockholders' equity at 2026-06-30, with $30M more due at the TYRVAYA close.
What is new since the last update is narrow and clinical. On 2026-08-20 Harrow released a prospective, randomized, contralateral-eye study in 50 patients (100 eyes) undergoing same-day bilateral intravitreal injections: patients reported approximately 58% less overall discomfort in the IHEEZO-treated eye than in the fellow eye given proparacaine 0.5%, statistically significant at all four post-injection assessments (all p≤0.048). Both eyes received 2% lidocaine-soaked pledgets first, so the comparison is IHEEZO versus a topical drop on top of a common base. That supports the retina channel behind IHEEZO's 65,477 Q2 units (+44% sequentially, +34% YoY), and it is a 50-patient patient-reported-comfort study — no label change, no reimbursement decision.
The narrative is maturing. What Would Change Our Mind
The structure to watch is the low-$40s shelf that has absorbed the guidance miss, the clinical release and the ratings reiterations. Losing it takes the story back to the pre-print range: a weekly close below $36 erases the post-Q2 stabilisation and leaves the mid-May reaction low near $34 as the next reference. On the fundamentals, any cut to the $350–365M revenue or $80–100M adjusted EBITDA range at or before the Q3 report ends the reaffirmation as a support for the tape. A financing that arrives before the H2 ramp is visible — an equity offering, ATM drawdown or new debt in an 8-K or prospectus supplement — changes the leverage picture directly. And if the TYRVAYA close slips past year-end, the H2 guidance that assumed a contribution from it loses a piece.
The read improves on the reverse: a Q3 print showing the sequential revenue step the guide implies, with adjusted EBITDA positive by a meaningful margin, converts the arithmetic problem into a delivered ramp.
