How to read this
The red mark is the published kill line — the price that would prove the thesis wrong. The dot is where the name closed on 9 October 2026; a dot left of the mark has closed through its line.
Distance is drawn on a square-root scale, so close calls get the room. Past 8% a row reads well clear, with a hollow dot to show the figure is off the drawn scale. Rows run tightest first.
Kill line
A weekly close below $6.36, the low of the 4 August 2026 post-report gap session, which would return the shares to their pre-report range. A Q3 2026 report that cuts the $72–75 million guidance or shows AVISE CTD price per test at or below $446 would also break the case.
Open pick Medium conviction, opened at $8.46. Since then −0.5% against the S&P 500's +0.5% (−1.0 pp). Kill line at open: $6.36 (weekly close); graded by at the latest.

Exagen's autoimmune-testing business is growing both test volume and price per test, and the shares closed at $8.17 on 8 October 2026, 31.7% below their 52-week high of $11.96. The third-quarter report, which Earnings Whispers lists for 11 November 2026 and Exagen has not yet confirmed, settles whether the raised full-year revenue guidance of $72–75 million holds.
The case still rests on the 4 August 2026 second-quarter results. Exagen reported revenue of $19.941 million against a $17.817 million consensus estimate and raised 2026 revenue guidance from $70–73 million (Benzinga; company 8-K). Its AVISE CTD lupus-related test averaged $446 per test over the trailing twelve months, up 4% year over year, on volume up 11% (company 8-K).
The narrative is maturing: the story is well known, and Exagen has issued no new business news since the 4 August 2026 report. The only fresh items since then are Benzinga's 17 September 2026 republication of the call transcript and the company's 20 September 2026 removal from the S&P Global BMI index (ad-hoc-news). The third-quarter report is the first new fundamental datapoint in more than three months.
Bullish and bearish views on Exagen Inc.
The bull case on Exagen Inc. (XGN), in brief: Pricing and volume rose together. The bear case: One test carries the revenue.
Bull case
- Pricing and volume rose together. AVISE CTD volume reached about 39,000 tests in Q2 2026, up 11% year over year, while the trailing-twelve-month price per test rose 4% to $446 (company release, 4 August 2026). The guidance raise depends on both continuing.
- The quarterly bar sits below the last result. Earnings Whispers listed third-quarter consensus at $18.28 million of revenue and a $0.19 loss per share as of 9 October 2026. Second-quarter revenue was $19.941 million (company release, 4 August 2026).
- Losses are close to breakeven. Q2 2026 adjusted earnings before interest, taxes, depreciation and amortization (EBITDA) was a $0.1 million loss, against a $1.7 million loss a year earlier. Gross margin was 61.3% (company release, 4 August 2026).
- Analyst targets sit above the price. On 5 August 2026 B. Riley raised its target to $12, TD Cowen to $10 and BTIG to $9, all keeping Buy ratings (Benzinga). MarketBeat put the average 12-month target of nine analysts at $9.625 as of 5 October 2026, with seven Buy, one Strong Buy and one Sell rating.
- A second test is on the calendar. On the August call, management placed myositis sales-force training at the end of the third quarter, ahead of a planned early-2027 launch (Q2 2026 call transcript, Benzinga, 17 September 2026).
Bear case
- One test carries the revenue. AVISE CTD produced $17.7 million of the $19.941 million Q2 2026 revenue (company release, 4 August 2026). A flat or lower price per test would weaken the main argument for the shares.
- Adjusted profit is not net profit. Exagen reported a $3.2 million net loss under generally accepted accounting principles (GAAP) for Q2 2026, with about $24.6 million of cash at 30 June 2026. Its adjusted EBITDA excludes stock compensation.
- Payers set the price. Medicare and commercial insurers decide what Exagen collects per test. The $446 figure depends on coverage decisions the company does not control.
- Passive ownership has shrunk. Ad-hoc-news reported that Exagen was dropped from the S&P Global BMI index on 20 September 2026. Index removal can reduce fund demand for a company with a market value of about $197.4 million (ad-hoc-news, 2 October 2026).
Price setup
The shares closed at $8.17 on 8 October 2026, after $8.19 on 6 October 2026 and $8.46 on 5 October 2026 (split/dividend-adjusted daily closes). They have traded between about $7.5 and $8.5 since late August: $7.92 on 21 August 2026, $7.58 on 4 September 2026, $8.41 on 18 September 2026 and $7.89 on 1 October 2026. The latest close sits in the upper half of that range.
The shares are up 97.3% over three months to 8 October 2026, most of it from the gap higher after the 4 August 2026 report. The low of that gap session was $6.36. The 14-day relative strength index (RSI) stood at 45.5 on 8 October 2026, a neutral reading, and the price is 31.7% under the $11.96 52-week high.
Crowding signs are limited. Company news flow has been quiet since August, the momentum reading is neutral, and the next report is roughly a month away. The index removal on 20 September 2026 is the one observable source of selling pressure that is not tied to the business.
Catalysts, next 30 days
- ~11 November 2026, est. Q3 2026 results. Earnings Whispers lists 11 November 2026 before the open; MarketBeat has listed 16 November 2026. Exagen reported Q2 on 4 August 2026 and Q1 on 11 May 2026. The report shows whether the $72–75 million guidance holds and whether AVISE CTD price per test stays above $446.
- ~1 January 2027, est. myositis test launch. Management's planned early-2027 launch is the second revenue source beyond AVISE CTD; the Q3 call should confirm sales-force training.

What would change the view
The case breaks if the third-quarter report cuts the $72–75 million full-year revenue guidance, or if the trailing-twelve-month AVISE CTD price per test comes in at or below $446. Either would mean the two drivers behind the August re-rating have stalled.
A delay of the myositis launch beyond early 2027 would weaken the case by leaving growth tied to one test. A Medicare or commercial coverage change that lowers AVISE CTD reimbursement would do the same.
On price, a weekly close below $6.36, the low of the 4 August 2026 gap session, would return the shares to their pre-report range and erase the post-report gain.
What it trades with
Exagen trades as a small, single-franchise diagnostics company with a market value of about $197.4 million (ad-hoc-news, 2 October 2026). Its moves since August have tracked its own report and index changes rather than broad medtech news. Removal from the S&P Global BMI index on 20 September 2026 reduces one link to passive fund flows. Reimbursement decisions by Medicare and commercial payers affect it alongside other specialty lab-testing companies.
Notes
- AVISE CTD generated $17.7M of $19.941M total Q2 2026 revenue; one test franchise carries the model.
- Adjusted EBITDA excludes stock compensation; Q2 2026 GAAP net loss was $3.2M against about $24.6M cash at 30 June 2026.
- Price per test is set by payers through Medicare and commercial coverage decisions.
- Exagen has not confirmed its Q3 2026 report date; Earnings Whispers lists 11 November 2026 and MarketBeat has listed 16 November 2026.
